ABOUT SNAKE RIVER ORTHOPAEDICS

DO NOT BRING THIS TO THE OFFICE.  THIS IS FOR YOUR INFORMATION ONLY.  Please read the following instructions to make it easier for us to take care of you.

  • New patients and patients who have not been seen for more than one year must complete a new set of patient information sheets.  Patients will need to re-sign the authorization to pay benefits form annually.   In addition, patients will be asked to verify their insurance and address on a regular basis.  If either of these change, please let our office know as soon as possible.
  • A patient who is being seen for a worker’s compensation claim must have an open and accepted claim before being seen by Dr. Kaltenbaugh.  Essentially, the patient is expected to follow up and make sure that their worker’s comp claim has been accepted and approved.

 

  • The insurance co-pay is expected to be paid at the time of service if applicable.  Dr. Kaltenbaugh is a specialist so, in some cases, the co-pay is higher than a regular primary care physician.  Snake River Orthopaedics accepts cash, check, Visa or Mastercard. If you are unable to pay the co-pay at the time of service, your appointment may need to be rescheduled.
  • In the event that surgery is needed, pre-authorization may be required from your insurance.  We will do our best to get the surgery authorized but if the insurance company does not get back to us in time for the surgery, the procedure may need to be rescheduled.
